THE CITY OF WEATHERBY LAKE
REQUESTS SEALED PROPOSALS FOR THE FOLLOWING CONSTRUCTION CONTRACT:
For the South Miami Water Main Relocation

INSTRUCTION TO PROPOSERS:

1. Sealed proposals for the South Miami Water Main Relocation for Weatherby Lake, Missouri, must be addressed to the Mayor of Weatherby Lake at 7200 NW Eastside Drive, Weatherby Lake, Missouri 64152 and be received before 10:00 a.m. on the date of closing.
2. Drawings, specifications and bidding requirements may be inspected at City Hall and copies may be obtained at the City Engineer's office: Ponzer-Youngquist, P.A., Inc., 227 E Dennis Ave, Olathe, KS 66061; phone # 913-782-0541. The non-refundable deposit for drawings and specifications shall be $50.00. The project consists of approximately 525 L.F. of 8” C900 PVC pipe, fittings, valves, fire hydrants, appurtenances and connections to system.
3. Pricing and anything pertaining to the RFP should be in a sealed envelope. It is preferred that the PROPOSAL RESPONSE FORM in this RFP be used. All RFP's must be sealed and marked on the outer envelope by RFP number and date of closing. The only information we will read at the closing will be the vendors, contractors and proposers who respond. The closing is at 10:00 a.m. on Tuesday, February 16, 2010 at City Hall.

Resolution Establishing Fees for the Provision of Health Services

WHEREAS, §§205.042 and 205.050 RSMo. (2000) and §192.300 RSMo. (2000) authorize the Health Center Board of Trustees to make and promulgate rules and regulations which will tend to enhance the public health and prevent the entrance of infectious, contagious, communicable or dangerous diseases into the county; and

WHEREAS, §192.300 RSMo. authorizes the Board of Health Center Trustees to establish reasonable fees to pay for the costs incurred carrying out such rules and regulations; and

WHEREAS, the Platte County Health Center provides several health services that enhance the public health and prevent the entrance of infectious, contagious, communicable or dangerous diseases into the county, including, but not limited to, a vaccination program for children and adults, primary care and well woman clinics for eligible indigent citizens, and walk-in clinic services for the residents of the county and other eligible persons; and

WHEREAS, the Board of Health Center Trustees has examined the cost to the Health Center of providing such services, including the cost of vaccine, medications, supplies, personnel, contract services, facilities expenses, and other expenses reasonably necessary to the services to be provided by the Platte County Health Center.

NOW THEREFORE, BE IT RESOLVED AS FOLLOWS:

1. The Platte County Health Center Board of Trustees shall establish fees for its health services and vaccines purchased by the Platte County Health Center and provided to citizens. The fees shall be based upon the then current actual cost incurred by the Health Center to provide the services or vaccine to citizens. The fees shall be listed on the “Health Services and Vaccine Fee Schedule”.

2. The fee for an office visit with a physician or advance practice nurse and the fee for contraceptives shall be charged on a sliding scale basis, depending upon the household income of the client. An administration fee shall be assessed for all office visits, except the administration fee will not be charged for services covered by Missouri HealthNet, Medicaid or Managed Care MC+. Fees for lab work, supplies, some medications and vaccines shall be adequate to cover the current cost of the items to the Health Center. The full cost of such items will be charged to the client, without reduction according to the sliding scale. However, the establishment of such fees shall not deny personal health services to those individuals who are unable to pay such fees or impede the prevention or control of communicable disease

The sliding scale shall be as follows:

Category F = Greater than 250% of federal poverty guidelines - 125% of Medicaid allowable charge
Category E = 201% to 250% of federal poverty guidelines - 90% of Category F payment
Category D = 151% to 200% of federal poverty guidelines - 80% of Category F payment
Category C = 126% to 150% of federal poverty guidelines - 50% of Category F payment
Category B = 101% to 125% of federal poverty guidelines - 30% of Category F payment
Category A = 100% or less of federal poverty guidelines - No cost
The fee shall be collected from the person receiving the services at the time of service.

3. The fees generated under this resolution shall be used by the Platte County Health Center to support the public health activities of the Health Center.

4. All fees for health services and vaccine shall be reviewed at least on an annual basis. Fees will be changed more frequently than annually if required by changes in costs from the Health Center's suppliers.

5. To obtain a copy of the current Health Service and Vaccine Fee Schedule, contact either Platte County Health Center facility 212 Marshall Road, Platte City, MO or 1201 East Street, Parkville, MO 64154. Platte County Parks & Recreation is seeking bids for printing of the 2010 Platte County Parks Calendar. In addition, the Platte County seeks bids that include mail house services. For consideration all individual components of the specifications must be addressed and one production contact person provided. Direct provider contractors preferred; any work associated with this project which is not performed in-house must be specified in the bid. RFP specifications are attached.
